Expiratory hold for total PEEP

An end-expiratory hold can estimate total PEEP only when the patient is passive and the circuit is sealed.

Auto-PEEP equals total PEEP minus set PEEP. Active expiratory effort or a leak can invalidate the maneuver.

Technique

  • Use the ventilator-specific expiratory-hold maneuver with RT when the patient is passive.
  • Confirm no significant leak or active expiratory effort.
  • Interpret total PEEP together with the flow-time scalar and the clinical picture.
